Apex Rehabilitation is seeking a Home Care Occupational Therapist to support clients in their homes, helping them regain independence and improve daily living activities.

Qualifications

• Must hold an OT degree from an accredited program.
• State licensure as an Occupational Therapist in good standing.
• Strong clinical and communication skills.
• Ability to adapt therapy plans for diverse client needs.
• Compassionate, patient-centered approach to care.
• Valid Driver’s License or reliable transportation.
• Valid Liability Insurance

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs.
• Develop and implement customized occupational therapy plans.
• Provide interventions to improve daily activity performance.
• Collaborate with clients, families, and healthcare professionals.
• Educate clients and families on adaptive techniques and equipment.
• Maintain accurate documentation of assessments and progress.
