Qualifications

• Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist.
• Strong clinical and interpersonal skills.

Responsibilities

• Conduct comprehensive assessments of clients in a home care setting.
• Develop personalized, goal-oriented physical therapy plans based on assessments.
• Administer therapeutic exercises, manual therapy, and other interventions.
• Collaborate with clients/families and other healthcare professionals for coordinated care.
• Educate clients on proper home exercise techniques and self-care.
• Maintain accurate documentation of assessments, interventions, and progress.
