Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist to assess and treat clients in their homes, focusing on independence and daily living activities. You will design personalized therapy plans and monitor progress to improve quality of life.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Valid Driver’s License and/or reliable transportation.
• Excellent communication and interpersonal skills.
• Ability to adapt therapy plans to client needs.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized occupational therapy plans to address clients' specific goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
• Educate clients and their families on adaptive techniques and equipment for increased independence.
• Maintain accurate and detailed documentation of assessments, interventions, and progress.
