Apex Rehabilitation is seeking a dedicated Home Care Physical Therapist to join our compassionate team and provide high-quality, in-home care. You will help clients regain mobility, manage pain, and improve well-being in the comfort of their homes.

Responsibilities include performing assessments, creating personalized plans, guiding exercises, and collaborating with families and other professionals. Prior home care experience is preferred, but new graduates are encouraged to apply.

Qualificações

• Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist.
• Strong clinical and interpersonal skills.
• Ability to adapt therapy plans to meet home care needs.
• Compassionate and patient-centered approach to care.

Responsabilidades

• Conduct comprehensive assessments of clients' physical conditions in a home care setting.
• Develop personalized and goal-oriented physical therapy plans based on assessment findings.
• Administer therapeutic exercises, manual therapy, and other interventions to enhance clients' functional abilities.
• Collaborate with clients, families, and other healthcare professionals to ensure coordinated care.
• Educate clients on proper techniques for home exercises and self-care.
• Maintain accurate and thorough documentation of assessments, interventions, and progress.
