Job summary

Providence PACE Seattle is seeking a Registered Nurse Care Manager to lead comprehensive care planning and coordination for participants, including assessment, implementation, and monitoring. You will work with an interdisciplinary team to ensure medically and financially appropriate care in the community.

The role requires an active WA RN license, BLS certification, and at least 1 year in long-term care or geriatric nursing.

Qualifications

• Washington RN license required at hire.
• BLS certification by American Heart Association at hire.
• 1 year long-term care or geriatric nursing experience.

Responsibilities

• Perform patient assessment, care planning, implementation, coordination, and monitoring.
• Coordinate interdisciplinary team care and medical management for recently discharged participants.
• Collaborate with IDT Social Worker and Physician to gather medical and social information.
