Job summary

970 Scotland Memorial Hospital, Inc is seeking an experienced Registered Nurse for hospice care. The role involves on-going patient assessments and development of individualized care plans, with regular home visits and on-call coverage as part of a dedicated interdisciplinary team.

The RN will perform prescribed treatments, coordinate with physicians, and ensure care aligns with state nursing practice acts, policies, and patient comfort goals.

Qualifications

• Licensed as a Registered Nurse (RN) in the United States.
• Minimum of 1 year in hospice or home health.

Responsibilities

• Provides nursing services under the Director of Clinical Operations with ongoing patient assessments.
• Makes regular home visits on assigned patients and coordinates care.
• Assists in providing 24-hour on-call services as scheduled.
• Performs prescribed treatments and procedures in compliance with the Nurse Practice Act.
• Conducts physician consults to obtain orders, updates the plan of care, and reviews progress.
