Job summary

Interim HealthCare RMC is seeking a Registered Nurse Case Manager (RNCM) to oversee patient care plans and coordinate with caregivers in home health settings. Conduct in-person interviews and assessments, manage care plans, and ensure optimal outcomes for patients and families.

You will educate families on wound care, IV therapy, and medications, collaborate with providers, and strive to reduce readmissions through preventative care and consistent care coordination.

Qualifications

• Active RN license in the state of the position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites and/or work locations.
• One year of professional RN experience in home health or similar setting; case management preferred.
• OASIS experience preferred.
• Trach/vent experience preferred, not required.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee the implementation and ongoing assessment of the patient's care plan with home health aides, LPNs, RNs, and other caregivers.
• Communicate patient conditions and collaborate with providers to adjust care as needs evolve.
• Educate patients and families on home health care procedures (wound care, IV administration, medication management).
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
