Job summary

Healthcare Services Group (HCSG) is hiring a Registered Dietitian to manage clinical nutrition services at a long-term care facility. You will assess resident needs, develop and document care plans, provide nutrition education, and collaborate with interdisciplinary teams to improve health outcomes.

Requires current RD/RDN and state licensure in good standing, strong organizational and communication skills, and knowledge of CMS regulations.

Qualifications

• Registered by the Commission on Dietetic Registration.
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of CMS regulations and nutrition care requirements (preferred).
• Strong organizational and communication skills.
• Compliance with vaccination policies as required by state/local regulations.

Responsibilities

• Assess the clinical nutrition needs of residents and develop and document the plan within the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical findings, labs, and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings as needed.
• All other duties as assigned.
