Job summary

Willamette Valley Behavioral Health is seeking a dedicated Case Manager (QMHA) to support clients in accessing resources, building stability, and navigating care systems. This full-time role offers flexibility in scheduling and a collaborative, client-centered environment.

You will connect clients with housing, medical care, benefits, and community resources while coordinating with clinicians and a multidisciplinary team to achieve treatment plan goals. EHR documentation is essential.

Qualifications

• QMHA certification required (or ability to obtain upon hire)
• Bachelor’s degree in social work, psychology, or related field
• Experience in behavioral health or community-based work preferred

Responsibilities

• Provide case management services in alignment with QMHA scope
• Connect clients with community resources (housing, medical care, benefits, etc.)
• Collaborate with clinicians and multidisciplinary team members
• Support clients in achieving treatment plan goals
• Maintain accurate, timely documentation in the EHR
• Advocate for clients and reduce barriers to care
• Conduct outreach when appropriate (community-based work may be included)
