Job summary

Personal Growth Counseling Services is seeking a Case Manager to oversee client cases and ensure services align with individualized treatment plans. You will use basic clinical interventions to support sobriety and mental wellbeing while maintaining up-to-date progress notes and collaborating with community resources.

Responsibilities include meeting productivity goals, auditing case files, coordinating referrals with agencies, and applying crisis de-escalation techniques across settings.

Qualifications

• High School Diploma or equivalent required.
• 3+ years of experience in the social work field.
• Chemical Dependency certification or Social Work licensure preferred.
• Certification in First Aid, CPR and De-escalation Training within 90 days of start.
• Must be 23 years of age or older with a valid driver's license.

Responsibilities

• Manage cases to ensure clients receive services per their ITP.
• Utilize basic clinical interventions to support sobriety and mental wellbeing.
• Maintain contact and referral information for community resources.
• Submit progress notes and documentation within 24–48 hours.
• Meet or exceed individual productivity goals.
• Review case files to ensure audit-ready status.
• Collaborate with referrals, agencies, and legal entities to coordinate care.
• Develop crisis de-escalation techniques for various settings.
• Perform duties in alignment with agency mission and standards.
• Complete additional duties as assigned.
