Job summary

Emblem Home Health & Hospice is seeking a Physical Therapy Assistant to deliver hands-on care under the supervision of a Physical Therapist, helping patients reach their goals with a flexible schedule that respects your life outside work.

This full-time PTA role serves across the East Valley, including Mesa, Tempe, Chandler and Gilbert, with responsibilities to guide therapeutic exercises, monitor progress, and maintain accurate notes in a collaborative, supportive team.

Qualifications

• Licensed PTA in Arizona (state license) is required.
• Graduate of CAPTE-accredited PTA program.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Deliver physical therapy treatments per PT and plan of care.
• Guide patients through therapeutic exercises and mobility training.
• Monitor and report patient progress and condition changes; educate on mobility aids.
• Maintain accurate clinical notes and participate in team meetings.

Education

Licensed Physical Therapy Assistant (PTA) — Arizona

Graduate of CAPTE-approved PTA program

Current CPR certification

Valid driver’s license
