Job summary

PACS Group is seeking a Staff Physical Therapist to evaluate and treat residents, document services, and educate families and staff. The role involves supervising PT assistants and collaborating with physicians and the health-care team to plan discharges and improve residents' mobility.

Responsibilities include developing personalized treatment plans, maintaining compliant documentation, and providing in-services to staff.

Qualifications

• Education and/or Experience: Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• License to practice as Physical Therapist in the state; continuing education per state rules.
• Proficient in computer data entry and electronic medical records (Net Health and Point Click Care).
• Good reading comprehension of procedures, policies, and manuals.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and support staff.
• Communicate with physicians, families, and team members about progress and discharge planning.
• Document accurately and timely according to regulations and payer requirements.
