Job summary

OneTeam Healthcare seeks a compassionate Physical Therapist (PT) to join our skilled nursing facility rehabilitation team in Sacramento, California. We offer Full-Time, Part-Time, or PRN schedules to fit your availability.

The PT will evaluate patients, develop individualized treatment plans, and provide skilled services to improve mobility, strength, balance, and independence, while collaborating with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current or eligible California Physical Therapist license
• Strong communication and documentation skills
• Ability to work effectively as part of an interdisciplinary rehabilitation team
• SNF or post-acute experience preferred

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and functional independence
• Establish measurable short- and long-term therapy goals
• Complete accurate and timely clinical documentation
• Collaborate with OT, SLP, nursing, physicians, and other members of the interdisciplinary team
• Educate patients, families, and caregivers regarding treatment and discharge recommendations
• Participate in care planning and discharge planning as appropriate
