South Pacific Rehab Services is hiring a Physical Therapist (PT) for a part-time position in a skilled nursing facility setting in Santa Rosa, CA. You will evaluate and treat patients, develop personalized care plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.

Responsibilities

• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
