Job summary

OneTeam Healthcare seeks a compassionate Physical Therapist (PT) to join our skilled nursing facility rehabilitation team in Sacramento, California. We offer Full-Time, Part-Time, or PRN schedules to fit your availability.

The PT will evaluate patients, develop individualized treatment plans, and provide skilled services to improve mobility, strength, balance, and independence, while collaborating with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current or eligible California Physical Therapist license
• Strong communication and documentation skills
• Ability to work effectively as part of an interdisciplinary rehabilitation team
• SNF or post-acute experience preferred

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and functional independence
• Establish measurable short- and long-term therapy goals
• Complete accurate and timely clinical documentation
• Collaborate with OT, SLP, nursing, physicians, and other members of the interdisciplinary team
• Educate patients, families, and caregivers regarding treatment and discharge recommendations
• Participate in care planning and discharge planning as appropriate

Setting: Skilled Nursing Facility (SNF)

We are seeking a compassionate and motivated Physical Therapist (PT) to join a skilled nursing rehabilitation team in Sacramento, California. We are flexible and can consider candidates interested in Full-Time, Part-Time, or PRN schedules.

This is an excellent opportunity for a Physical Therapist who enjoys working with the geriatric and post-acute population and wants flexibility while being part of a supportive rehabilitation team.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and functional independence
