We are seeking a Physical Therapist to join a collaborativeskilled nursing facility (SNF) team in Sacramento, CA. This opportunity isideal for a clinician who enjoys helping patients improve mobility, strength,balance, safety, function, and independence through individualized,evidence-based care.
Evaluate patients and develop individualizedplans of care based on functional goals.
Provide evidence-based therapeutic exercise,mobility, balance, gait, and functional training.
Monitor patient progress and modify treatmentplans as clinically appropriate.
Educate patients, families, and caregivers onsafety and home programs.
Complete accurate and timely clinicaldocumentation.
Collaborate with physicians, nursing,rehabilitation professionals, and the interdisciplinary team.
Graduate of an accredited Physical Therapyprogram.
Active CA Physical Therapist license in goodstanding, or eligibility for licensures.
Current BLS/CPR certification as required by theemployer.
Strong clinical reasoning, communication,documentation, and teamwork skills.
Experience in the applicable setting preferred;qualified clinicians at varying experience levels are encouraged to apply.
Competitive compensation: $60 - $70 per hour
