Job summary

Luxor Healthcare is seeking a Physical Therapist (PT) to provide patient care and conduct assessments in line with facility policies. The role supports physician orders and care plans while documenting progress and facilitating discharge planning.

ThePT will collaborate with the care team, educate patients and families, and ensure effective communication across the healthcare team to optimize outcomes.

Qualifications

• Active Physical Therapist certification required.
• Missouri licensure as PT is mandatory.
• Experience with patient assessment and care planning preferred.

Responsibilities

• Provide patient care and assessment per established standards and policies.
• Follow physician orders and care plans developed by evaluating therapist.
• Document patient status and progress to support care effectiveness.
• Collaborate with the team to optimize patient outcomes and discharge planning.
• Educate patient and family as directed by the evaluating therapist.
• Maintain communication with the patient and care team for continuity of care.
