Responsibilities

• Provides patient care and assessment per practice standards and policies.
• Delivers care per physician orders and plan of care developed by evaluating therapist.
• Takes measurements of patient status within licensure limits to assess care effectiveness.
• Maintains communication with evaluating therapist to maximize benefits and summarize progress.
• Implements education plan and identifies additional educational needs with evaluating therapist.
• Follows discharge plan and assists with discharge planning and goal reassessment.
• Communicates with patient and healthcare team to promote maximum care.
• Demonstrates proper use of clinical techniques, procedures and equipment.
