Qualifications

• Master's or Doctoral degree in Speech-Language Pathology required.
• Licensed Speech-Language Pathologist in the state of service.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong clinical, documentation, and communication skills.
• Must maintain continuing education requirements.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide speech, language, cognitive, and swallowing therapy.
• Monitor progress and modify treatment plans as needed.
• Communicate goals and outcomes with physicians, nursing staff, residents and families.
• Complete accurate documentation in compliance with regulations.
• Participate in care planning and discharge planning activities.
