Job summary

Wyomissing Health & Rehabilitation Center in Reading, PA is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. You will provide skilled PTA services under the supervision of a licensed PT in a SNF and long-term care setting, focusing on restoring mobility and independence for residents.

Ideal candidates hold PA PTA licensure or indirect license eligibility, have strong communication and teamwork skills, and are eager to work in a resident-centered,

Qualifications

• Current Pennsylvania Physical Therapist Assistant license or Indirect License eligible.
• Graduate of an accredited Physical Therapist Assistant program.
• SNF or Long-Term Care experience preferred, but new graduates are encouraged to apply.
• Experience with Net Health is a plus.
• Strong communication, organization, and teamwork skills.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
• Assist residents in improving mobility, balance, strength, endurance, gait, and overall functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Educate residents and caregivers on therapeutic exercises, mobility techniques, and fall prevention strategies.
• Complete accurate and timely documentation using Net Health.
• Collaborate with nursing staff, physicians, and the interdisciplinary care team to achieve the best possible resident outcomes.
• Maintain compliance with all federal, state, and facility regulations.
