Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant to provide geriatric therapy during home visits in Sandy Springs, GA. You will work PRN, selecting assignments that fit your availability and maintain independence in residents’ daily activities.

Responsibilities include delivering skilled PT under a plan of care, documenting thoroughly, and collaborating with physicians and care teams to optimize mobility and function for aging residents.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Active GA PTA license.
• Two years of clinical experience preferred (not required).

Responsibilities

• Provide skilled physical therapy treatments under the plan of care
• Help patients improve mobility, function, and independence
• Communicate effectively with physicians, families, and the care team
• Complete accurate, timely documentation
