Responsibilities

• Perform physical therapy evaluations and establish individualized treatment plans
• Provide skilled interventions for mobility, balance, strength, gait, and functional performance
• Monitor resident outcomes and modify treatment plans when appropriate
• Collaborate with PTAs, nursing, and other rehabilitation professionals
• Complete accurate clinical documentation
