Job summary

Aventura at Oakwood Village in Springfield, OH is seeking a dedicated PRN Physical Therapist to join our rehabilitation team. You’ll evaluate residents, develop individualized plans, and help restore mobility, strength, balance, and independence in a skilled nursing and long-term care setting.

This role offers flexible PRN scheduling with coverage across weekdays, weekends, holidays, and vacations. Net Health and PCC documentation systems are used; strong communication and teamwork are

Qualifications

• Active Ohio Physical Therapist license.
• Experience in SNF/LTC preferred but not required.
• Experience with Net Health and PCC preferred.
• Strong communication and clinical skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services focused on mobility, strength, balance, and functional independence.
• Accurately document evaluations, treatments, and progress in Net Health and PCC.
• Collaborate with nursing, therapy, and interdisciplinary teams.
• Maintain compliance with facility and regulatory standards.
