Job summary

The Care Team Hospice is seeking a compassionate Physical Therapist (PT) to support hospice patients in the Greater Youngstown Area and surrounding communities on a PRN, paid-per-visit basis. This role offers flexible scheduling and meaningful one-on-one patient care, with opportunities to supplement income while applying clinical expertise in a home- and facility-based setting.

• Active Ohio PT license required.
• Graduate of an accredited Physical Therapy program.
• Hospice/home health/geriatric experience preferred.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently and in patients' homes and facilities.

Responsibilities

• Complete PT evaluations focused on mobility, strength, balance, safety, and comfort.
• Develop and implement hospice-focused interventions aligned with plan of care and goals.
• Recommend assistive devices, safe transfer techniques, and environmental modifications.
• Educate patients, families, and caregivers on mobility, fall prevention, energy conservation, and safe daily activities.
• Collaborate with the interdisciplinary hospice team and complete accurate clinical documentation.

The Care Team Hospice is seeking a compassionate and experienced Physical Therapist (PT) to support hospice patients throughout the Greater Youngstown Area and surrounding communities on a PRN, paid-per-visit basis.

This is a great opportunity for a Physical Therapist looking to supplement their current income, maintain flexibility, or use their clinical expertise in a more personalized, patient-centered setting.

Unlike traditional rehabilitation settings, hospice physical therapy focuses on comfort, safety, mobility, and quality of life rather than aggressive rehabilitation goals. You'll work one-on-one with patients and families, helping them maintain independence where possible, improve comfort, and navigate everyday activities safely.

We're looking for a PT who is compassionate, dependable, clinically confident, and comfortable working independently while collaborating with an experienced hospice team.

Key Responsibilities

• Complete individualized physical therapy evaluations focused on mobility, strength, balance, safety, functional abilities, and patient comfort.
• Develop and provide appropriate therapy interventions aligned with each patient's hospice plan of care and personal goals.
• Recommend assistive devices, safe transfer techniques, positioning strategies, and environmental modifications to improve comfort and safety.
• Educate patients, families, and caregivers on mobility, fall prevention, energy conservation, positioning, and safe daily activities.
• Collaborate with the interdisciplinary hospice team and complete accurate, timely clinical documentation.

Qualifications

• Active and unrestricted Physical Therapist (PT) license in Ohio required.
• Graduate of an accredited Physical Therapy program.
• Previous physical therapy experience required; hospice, home health, palliative care, or geriatric experience preferred.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently and provide care in patients' homes and facilities.
• Experience supporting patients with serious or life-limiting illnesses preferred.
• Valid driver's license, reliable transportation, and current automobile insurance required.
