Sobe Innovative Rehab in Stuart, FL seeks a licensed Physical Therapist to provide hospice-focused care in home/ALF settings. Work PRN with flexible assignments and clinical autonomy.

Qualifications

• Active Florida Physical Therapist (PT) license.
• Degree in Physical Therapy from an accredited program.
• New graduates and experienced therapists encouraged to apply.

Responsibilities

• Evaluate and treat residents in a home/assisted living setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
