Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services at an Assisted Living Facility and for Hospice Patients in a home setting in Melbourne, Florida.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR
