Job summary

Uniontown Nursing and Rehab in Uniontown, Pennsylvania, is seeking a compassionate PRN Physical Therapist to join our in-house rehabilitation team. The role focuses on mobility, strength, balance, safety, and functional independence for residents in a Skilled Nursing and Long-Term Care setting.

The ideal candidate holds a PA PT license or is eligible, has a PT degree from an accredited program, and demonstrates strong communication and clinical judgment.

Qualifications

• Current Physical Therapist license in Pennsylvania or eligibility for licensure
• Graduate of an accredited Physical Therapy program
• Strong communication and clinical decision-making skills
• Compassionate and resident-focused approach to care
• Ability to work effectively as part of an interdisciplinary team
• Skilled Nursing/LTC experience is a plus

Responsibilities

• Evaluate residents and develop individualized plans of care
• Provide skilled physical therapy services focused on mobility, strength, balance, safety, and functional independence
• Establish measurable goals and monitor resident progress
• Complete evaluations, progress reports, reassessments, and discharge documentation
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary care team
• Educate residents, families, and caregivers regarding treatment plans and safety
• Maintain accurate and timely clinical documentation using Net Health and PCC
• Provide coverage based on facility and resident needs
