Job summary

Legacy Care is seeking a compassionate physician to provide PRN coverage for our Richmond-area post-acute care practice. You will deliver onsite primary, acute, and chronic care to medically complex adults in skilled nursing, rehab, and long-term care settings, collaborating with APPs and care teams.

The role emphasizes clinical autonomy, flexible scheduling, and improving outcomes while supporting transitions of care and reducing avoidable hospitalizations.

Qualifications

• MD or DO with an active Virginia medical license.
• Active DEA registration is required.
• Board certification or eligibility in Internal Medicine, Family Medicine or Geriatrics.

Responsibilities

• Provide PRN coverage for assigned Richmond-area facilities.
• Deliver onsite primary, acute and chronic care to post-acute and long-term care residents.
• Evaluate changes in condition and manage complex cases.
• Collaborate with nurse practitioners, nursing teams and facility leadership.
• Establish person-centered goals of care with patients and families.
• Support utilization, quality improvement and regulatory initiatives.
• Complete timely clinical documentation.
• Promote smooth transitions of care across the continuum.
