Job summary

CareCore at Willowood in Brunswick, OH, is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team. This role offers flexible weekday and weekend opportunities in a skilled nursing and long-term care environment.

You will perform comprehensive PT evaluations, develop personalized plans, and work with a collaborative interdisciplinary team to enhance residents’ mobility and independence while ensuring compliant documentation and using Net Health and PCC.

Qualifications

• Current Ohio Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Skilled nursing or long-term care experience preferred, but not required.
• Excellent communication, documentation, and clinical decision‑making skills.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based therapy services to improve mobility, strength, balance, and independence.
• Collaborate with physicians, nursing staff, therapy team, residents and families.
• Complete accurate and timely documentation using Net Health.
• Utilize PCC as part of the care process.
• Maintain compliance with federal, state, and company regulations.
