Job summary

CareCore at Willowood in Brunswick, OH is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team. This role offers flexible scheduling in a skilled nursing and long-term care setting, with a focus on mobility, balance, and functional independence for residents.

You'll evaluate patients, develop personalized treatment plans, document using Net Health, and collaborate with physicians, nursing and therapy staff. PRN hours are available with potential for additional shifts.

Qualifications

• Current Ohio Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• SNF/long-term care experience preferred.
• Excellent communication, documentation, and clinical decision-making skills.
• Experience with Net Health and PCC is preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide evidence-based therapy services to help residents improve mobility, strength, balance, and functional independence
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Complete accurate and timely documentation using Net Health
• Utilize PointClickCare (PCC) as part of the interdisciplinary care process
• Maintain compliance with all federal, state, and company regulations
