Hillsboro Post Acute is seeking a skilled Physical Therapist (PT) to join our therapy department on a PRN basis. We are dedicated to helping residents recover, regain independence, and return to their highest possible level of function.

The PRN PT will perform comprehensive evaluations, develop individualized treatment plans, deliver evidence-based interventions, and closely monitor progress while coordinating with nursing and medical staff.

Qualifications

• Must hold current Physical Therapist license in Ohio.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing, rehab, or post-acute care preferred.
• Strong clinical reasoning and communication skills.
• Dedication to providing exceptional resident care.

Responsabilités

• Perform comprehensive evaluations and establish individualized treatment plans.
• Deliver evidence-based physical therapy interventions.
• Assess resident progress and modify treatment plans as needed.
• Work closely with nursing staff, physicians, and rehabilitation team members.
• Complete timely documentation in accordance with company and regulatory standards.
