Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist for a PRN role in a Skilled Nursing Facility. You will evaluate movement limitations, create individualized plans, and lead therapy sessions focused on restoring function and quality of life.

You’ll work with an interdisciplinary team to drive outcomes, stay current with PT best practices, and maintain compliance with standards. Flexible scheduling supports work–life balance and ongoing professional development.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred; clinical experience preferred; new grads welcome.

Responsibilities

• Perform comprehensive PT evaluations and develop personalized treatment plans.
• Provide hands-on therapy including manual techniques, exercise, and mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track progress and adjust plans; collaborate with physicians, nurses, OTs and therapists.
