Job summary

Innovate Rehab and Wellness in Moberly, MO is seeking a PRN Licensed Physical Therapist to provide skilled nursing facility services. You will evaluate patients, develop individualized treatment plans, and work with an interdisciplinary team to restore movement and quality of life.

The role emphasizes one-on-one care, flexible scheduling, and ongoing professional development within a therapist- and veteran-owned company that values clinical outcomes and patient satisfaction.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
