Job summary

Innovate Rehab and Wellness is seeking a Physical Therapist for a PRN, on-site role in a skilled nursing facility. You will evaluate patients, design individualized treatment plans, and deliver hands-on therapy to restore movement and function.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Valid state PT license.
• CPR certification preferred.
• Clinical experience preferred; new grads welcome.

Responsibilities

• Perform comprehensive PT evaluations and develop personalized treatment plans.
• Provide hands-on therapy, manual techniques, therapeutic exercise, and mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians to optimize recovery.
• Stay current with PT best practices through continuing education and regulatory compliance.
