Job summary

Sobe Innovative Rehab in Palm Bay, FL is seeking a licensed Physical Therapist to provide hospice therapy in a home setting. Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
