Sobe Innovative Rehab in Polk City, FL is seeking a licensed Physical Therapist to provide hospice therapy in a home setting, helping older adults maintain independence and comfort.

This PRN role offers a flexible schedule, treatment in a single location, and the chance to work with a supportive interdisciplinary team. You will evaluate residents, develop plans, document in the EMR, and collaborate with physicians and families.

Qualifications

• Licensed Physical Therapist in Florida.
• Education: PT degree (Bachelors/Master/Doctorate).
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.

Education

Physical Therapy degree (Bachelor/Master/Doctorate)

Active Florida PT license
