Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. Work with a compassionate team to enhance independence and quality of life for older adults in Fort Walton Beach, FL.

This PRN opportunity offers flexible assignments, autonomy in clinical decisions, and competitive pay. You’ll collaborate with physicians and families while maintaining thorough EMR documentation and patient progress notes.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
