Qualifications

• Licensed Florida PT in good standing.
• Graduate of accredited Physical Therapy program.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate EMR documentation.

Education

PT degree (Bachelor/Master/Doctorate) in Physical Therapy
