Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a PRN home health role serving adults and seniors across South Dallas and surrounding TX areas. You will perform in-home evaluations, develop care plans, and deliver one-on-one therapy focused on mobility and safety.

The role offers autonomy, flexible daytime scheduling, and collaboration with an interdisciplinary team to support patient independence in the home setting.

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligible Compact PT Licensure.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Knowledge of OASIS and home health regulations preferred.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care based on patient goals and physician orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical and complex cases.
• Deliver evidence-based interventions for mobility, gait training, balance, and functional independence.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and modify treatment plans as needed.
• Document accurately in the EMR at the point of care.
• Collaborate with physicians, nurses, case managers, and the care team.
• Supervise PTAs and home health aides per state regulations.
