Job summary

Creative Solutions In HealthCare is seeking a compassionate Occupational Therapist to join our in-house rehab team on a PRN basis. You will assess residents to improve independence and safety through meaningful daily activities and functional outcomes.

The ideal candidate holds an active Texas OT license, SNF or long-term care experience, and strong communication to collaborate with the team and educate patients and families.

Qualifications

• Must hold an active Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy practice and documentation.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Develop individualized treatment plans to maximize independence in ADLs and IADLs.
• Assess home environments and recommend modifications.
• Provide training on adaptive equipment and safety.
• Ensure treatments align with care plan and regulations.
• Monitor response to treatment and adjust as needed with the rehab team.
• Educate patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Complete documentation thoroughly and timely per policies.
