Job summary

Qualifications

• Must hold an OT license in Ohio.
• Experience in skilled nursing or long-term care preferred.
• Familiar with Net Health and PCC preferred.
• Strong clinical, communication, and documentation skills.
• Ability to work independently while collaborating with the rehab team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled OT services to short-term and long-term residents.
• Collaborate with an interdisciplinary team to maximize outcomes.
• Complete timely and accurate documentation using Net Health.
• Maintain compliance with state, federal, and company regulations.
