Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities in Fredericksburg and surrounding communities. You will evaluate patients in their homes, develop individualized plans of care, and help patients regain independence in activities of daily living.

Offer flexible scheduling and territory selection. You will cover the areas you choose, with mileage reimbursement and consistent patient opportunities, all while delivering one-on-one care

Qualifications

• Active Virginia Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.
• WellSky experience is a plus.

Responsabilités

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve independence with activities of daily living.
• Provide therapeutic interventions to improve strength, coordination, mobility, and functional ability.
• Assist patients with dressing, bathing, grooming, toileting, meal prep, and other daily activities.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients, families, and caregivers.
• Monitor progress and adjust treatment plans as needed.
• Complete reassessments and discharge planning.
• Coordinate care with physicians, nurses, therapists, and other care-team members.
• Complete accurate documentation using WellSky.
