Job summary

Human Touch Home Health seeks a compassionate Occupational Therapist (OT) for PRN home health opportunities in Loudoun County, Virginia. You’ll choose the territories to cover, building a caseload around your availability and visiting patients in Leesburg, Ashburn, Sterling, Purcellville and nearby communities.

You’ll evaluate patients in their homes, develop care plans, and provide interventions to improve independence in daily living, mobility, and safety, while documenting progress and

Qualifications

• Active Virginia Occupational Therapist License required.
• Graduate of an accredited Occupational Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health experience preferred, but not required.
• WellSky experience is a plus.

Responsibilities

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve independence with activities of daily living.
• Provide therapeutic interventions to improve strength, coordination, mobility, and function.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on safe techniques and strategies.
• Monitor progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapists, and other care team members.
• Complete accurate and timely documentation using WellSky.
• Participate in reassessments and discharge planning.
