Job summary

Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities throughout Loudoun County, Virginia. You will complete occupational therapy evaluations in patients' homes, develop individualized plans of care, and help patients improve independence with activities of daily living.

Qualifications

• Active Virginia Occupational Therapist License required.
• Graduate of an accredited Occupational Therapy program.
• Valid Driver's License and reliable transportation.
• WellSky experience is a plus.
• Home Health experience preferred, but not required.

Responsibilities

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve independence with activities of daily living.
• Provide therapeutic interventions to improve strength, coordination, mobility, and function.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on safe techniques and strategies.
• Monitor progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapists, and other members of the care team.
• Complete accurate and timely documentation using WellSky.
• Participate in reassessments and discharge planning.
