Job summary

Qualifications

• Current, active, and verifiable LPN license issued by the Pennsylvania State Board of Nursing.
• Provide safe nursing care within LPN scope and state regulations.
• Knowledge of medication administration, infection control, and resident safety.
• Recognize changes in resident condition and communicate concerns promptly.

Responsibilities

• Provide nursing care per physician orders and resident care plans.
• Administer medications and treatments per protocol.
• Monitor residents, obtain vital signs, and recognize changes.
• Report significant changes to the supervising nurse or physician.
• Perform nursing assessments, wound care, and dressing changes as authorized.
• Maintain accurate documentation in resident records and EHR.
• Follow infection control, fall prevention, and safety procedures.
• Communicate with residents, families, physicians, and care team.
