Windsorhh is seeking a PRN Home Health Social Worker to support patients in the comfort of their homes in Youngstown, OH. You will provide compassionate counseling, assess needs, and help families navigate resources.

This role offers flexible PRN scheduling, a supportive team, and meaningful one-on-one patient care while contributing to quality outcomes in home health. Collaboration with physicians, nurses, and therapy staff ensures coordinated care and promotes patient independence.

Qualifications

• Licensed Social Worker (LSW) is required; MSW preferred.
• Home health experience preferred, but not required.
• Excellent communication and organizational skills.
• Compassionate, patient-focused mindset.
• Reliable transportation and valid driver's license.
