Stern at Home Therapy in Marietta, GA is seeking a PRN Physical Therapist to provide one-on-one home health care to geriatric patients across Marietta and surrounding communities.

You will have clinical autonomy, a flexible schedule, and support from an administrative team handling referrals, authorizations, and patient coordination, with compensation per visit of $72 for follow-ups and $85 for evaluations.

Qualifications

• Active Physical Therapist license in Georgia required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation required.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently in a home-based setting.
• Home health or geriatric experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients' homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
• Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and the interdisciplinary care team.
