ALC Home Health is seeking a PRN Physical Therapist to deliver in-home care across Illinois. The role offers competitive per-visit rates ($75 for standard visits, $85 for evaluations) and a $2,500 sign-on bonus, with flexible scheduling and administrative support to track visits via Emergency Medical Response technology.

Responsibilities include evaluating patients, directing therapy, educating families, documenting progress, and coordinating with the Home Health Director.

Formación

• Degree in Physical Therapy from an accredited program.
• Licensed to practice as a physical therapist in the state.
• Two years of experience; home health preferred.
• Good verbal and written communication; well organized.
• Current CPR certification.
• Driver's license with insured vehicle.

Responsabilidades

• Provide physical therapy services per physician plan of care.
• Assess mobility, ROM, strength; develop plan of care.
• Instruct patients/families in therapy goals and devices.
• Refer to other services as needed and prepare progress summaries.
• Discharge planning and quarterly progress summaries.
