Qualifications

• Requires a degree in physical therapy and state licensure.
• Two years of relevant PT experience preferred.
• Strong communication and organizational skills.
• CPR certification is required and valid.
• Must hold a valid driver's license with insured vehicle.

Responsibilities

• Provide physical therapy services per physician plan of care.
• Assess functional status and home environment for rehab potential.
• Direct PT treatment, instruct patients and caregivers.
• Identify needs for other home health services and refer as needed.
• Prepare progress summaries and discharge planning.
• Provide PT consultation to families and caregivers as indicated.
• Supervise aides per policy and state requirements.
• Instruct and evaluate home health aides every two weeks when PT is sole skilled service.
