HealthPro Heritage in Fort Wayne, IN is seeking a PRN Physical Therapist to join our therapy team. This flexibly scheduled role supports vacations, sick days, and census fluctuations at a highly rated assisted living facility.

You will assess patients, design and implement therapeutic exercises, educate families, and document progress while collaborating with a multidisciplinary team to deliver compassionate care and measurable outcomes.

Qualificações

• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills with a patient-centered approach.
• Ability to lift and assist patients as needed.

Responsabilidades

• Patient Assessment: Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and other treatments to improve function and reduce pain; educate patients and families on exercises and home routines.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans.
• Collaboration: Work with healthcare providers and participate in multidisciplinary meetings as needed.
